Mental Health Rehabilitation Services operates in Southern Adelaide.

Service provision includes:
Clients receive individualised one on one support from a Support Worker, are encouraged to identify particular needs/strengths, and develop their own goals to facilitate engagement in an enhanced quality of life and full citizenship. Service is offered on an outreach basis and is flexible according to the client’s needs.

PARTNERSHIP:
Centacare provides non-clinical support to clients, and care is shared with the Southern Adelaide Health Services (SAHS) Mental Health system which provides ongoing clinical support.

ELIGIBILITY AND REFERRALS:
Public Mental Health staff identify adults with complex needs who may want to consider Centacare services. These people are then approached and if they agree, they accept the service on a voluntary basis.
