Mental Health Rehabilitation Services


Southern Region of Adelaide and Northern Country Regions

SERVICE
These services provide support to adults in the southern region of Adelaide and northern country region incorporating Barossa, Burra, Clare and Gawler, and who are living with a mental illness. Northern offices are located at Yorketwon Road Elizabeth and Main Road Clare. The services are funded by the Health Department of South Australia, and operate within a recovery, client centred approach.

Clients receive individualised one on one support from a Support Worker, are encouraged to identify particular needs/strengths, and develop their own goals to facilitate engagement in an enhanced quality of life and full citizenship. Service is offered on an outreach basis and is flexible according to the client’s needs.


PARTNERSHIP
Centacare provides non-clinical support to clients, and care is shared with the Southern Adelaide Health Services (SAHS) Mental Health (in the South) system and Wakefield Health Region Mental Health (in the North), which provides ongoing clinical support.

ELIGIBILITY AND REFERRALS
Public Mental Health staff identify adults with complex needs who may want to consider Centacare services. These people are then approached and if they agree, they accept the service on a voluntary basis.
