Your LINC Mental Health Services

Your Linc is named for its focus on Lived, Identity, Navigate and Connect, We offer community-based individual psychosocial support.

The service is delivered through a partnership between Centacare Catholic Family Services and KWY Aboriginal Corporation.

Your LINC embodies a strength-based approach to mental health support, where our workers walk alongside individuals to reclaim their sense of self and community.

This service is funded by the Department of Health and Wellbeing.

Our supports

Our mental health workers provide person-centred psychosocial support to individuals and their families/carers, where appropriate. 

This includes:

• goal setting, 
• application and transition to NDIS services.

KWY Aboriginal Corporation offers a dedicated and community-controlled Aboriginal service, centering on cultural ways of healing, engagement, and recovery.

Eligibility

Our service is for people aged 16 and older who are clients of the mental health community teams or people within this age group who may have accessed the Urgent Mental Health Care Centre, hospital inpatient units, or emergency departments.

Referrals

We accept:

• referrals from Urgent Mental Health Care Centre,
• referrals from community teams, hospitals and emergency departments.
